Brehm Medical Center offers a fully integrated Patient Portal and Electronic Medical Records (EMR) system to provide a better, more accurate and efficient patient experience. We can record and reference your medical records to check for potential drug interactions for safer care and give you a more personalized treatment plan.

If you are a current Brehm Medical Center patient we encourage you to use our secure Patient Portal. The portal allows you to request appointments, get lab results, make payments, and much more.

If you are a new patient, we ask that you complete the forms below in advance of your first appointment. Please bring the completed forms below with you to your first appointment:
